Pavement tragedies brought up by Weepej with the following links that he provided, but moved from previous thread in Cycling.

All three incidents have little to go on really.

Until there are crash investigations we can only go on the possibilities of distraction, mechanical failure, failing to look properly, loosing control of the car, or deliberate action.

The first case says that they have ruled out deliberate action, whilst I'd expect this too in most cases of this type, it must happen!
I'd assume at this point that it is more likely to be, distraction leading to loss of control.
All road deaths are sad and many tragic, but every case will be a bit different.
The third has perhaps a stronger possibility of mechanical failure from an altered car but it's impossible at this stage to know.
There is a hit and run although the chap driving was found and is out on bail.
Reminds to all road users that leaving the scene of an accident can carry a very hefty penalty all by itself.
